Application for NEW Students:

Our Enrollment Application for the 2016-2017 school year will run January 19, 2016 - March 4, 2016. Please make sure to have your application turned in by 3:30 PM the last date of the application period.

You may access the application by clicking on the Online Registration. This application is only required for NEW students that wish to attend Monticello Academy. Kindergarten applicants must be 5 years of age by September 1, 2013.

The next Orientation will be held:

February 3, 2016 6:30 - 7:30 PM
February 25, 2016 6:30 - 7:30 PM

 

Please do not contact the office with questions regarding enrollment until AFTER visiting our Admissions Frequently Asked Questions (FAQs) page (you may go there directly by clicking here) and/or attending one of our Orientations listed above where you will be able to speak to an administrator to answer your questions.
 


Registration for RETURNING Students

This section is only for students who are currently registered and attending Monticello Academy and are planning to return next year.

Students already attending Monticello Academy are pre-enrolled the following year unless they are withdrawn, and need not participate in Open Enrollment and the Lottery. Parents wishing to re-enroll their currently registered and attending students need only submit a Letter of Intent Form to inform the school of plans to return and participate in student registration during the summer.

Although enrollment for students currently attending Monticello Academy is confirmed for the next school year, all forms are still due at the school office by the given deadlines. If Monticello Academy does NOT receive your Letter of Intent and registration forms, the school will assume your family is not returning for the following school year and will provide you with a withdrawal form.

The law requires that all children be current on their immunizations or have an exemption granted by the Health Department. Students who are not in compliance with this requirement may not be allowed to attend school. Parents of incoming 7th grade students in particular should be aware that there may be additional immunization requirements for their child. Please check the most current Immunization Requirements and make arrangements to update any needed immunizations prior to the fall.

As a reminder, if your plans should change and you no longer wish for your student to attend Monticello Academy, we would appreciate such notification as soon as possible so that we may anticipate new enrollment and staffing needs.  Additionally, state law requires that parents notify their school of attendance no later than June 30 of their transfer to another school.  Should you desire to transfer after June 30, you are required by law to provide Monticello Academy with a letter of acceptance for enrollment from your student's new school.

Step 1: Letter of Intent forms (Currently - March 25, 2016)

Complete and submit the Letter of Intent Form to notify the school if your student plans to return in the fall.  This will ensure his/her spot is not released and offered to a new applicant through the lottery.  This form is due on or before
March 25, 2016.

Step 2: (August)


Complete the online Registration Packet which contains annual notices and important forms required to update your student's information each year.

In order to register your new student at Monticello Academy, you should have turned in the following information with your Commitment to Enroll Form:

  • Original Birth Certificate – The state requires that we see the original birth certificate, then make a copy for the student’s file. Please make sure your student’s birth certificate is accessible, or if necessary, make arrangements to obtain one as soon as possible. If your child's original birth certificate is not in English you must provide an English translation.
  • Current Immunization Record – The law requires that all students be current on their immunizations or have an exemption granted by the Utah Department of Health.  Parents of incoming 7th grade students in particular should be aware that there may be additional immunization requirements for their student.  Please check the most current Immunization Requirements and make arrangements to update any needed immunizations prior to the fall.
  • Proof of Vision Screening (Kindergarten – 3rd Grade)
  • Special Education Information if you have it (if applicable) – If your student has been serviced in Special Education we will need a current copy of the IEP or 504 so that our student services team can address the needs of your student.  (Monticello Academy will request records from your student’s current school upon registration.)
  • Proof of Custody/Guardianship (if applicable) – Only a parent or guardian with “physical custody” can register a student.  In the case of divorce, a copy of the divorce agreement, signed by a judge, is required to establish physical and custodial rights.  
  • Copy of withdrawal form, if possible, from the school student was most recently registered – Students may only be registered in one school at a time. You are required to take your letter of acceptance to your child's previous school and officially withdraw your child from that school. Monticello Academy cooperates with district and charter schools to ensure that students are not dually enrolled.
  • Completed Registration Forms – The necessary registration forms are available online in August.

Please note that any student who has been expelled from any school, including Monticello Academy, during the preceding 12 months may be denied enrollment. (Utah State Code 53A-11-904(3)).
