Office Renovations - Contact Information

Our office is currently under renovation. At this time we are unable to accept faxes. Documents may be emailed to info@monticelloacademy.net.

For the next few days our office staff will be available by phone and email  from 9:00 am to 3:30 pm.

There should be an office staff member at the building for the food donatiions today and next week. 

We appreciate your patience while we are temporarily out of our normal workspace.

Breakfasts for Teacher Training - Parent Council Sponsored
We are so excited to celebrate coming back to school but did you know our teachers come back two weeks ahead! As a parent council would like to provide breakfasts to our teachers the week before school starts.  We are looking for voluntary donations.
 
Please remember time spent shopping and preparing food can be recorded as volunteer hours. Please check your email for links to the sign ups. As always we appreciate your support of the parent council. 
 
Food donations need to be brought to the school the day ahead to ensure we have all items for the following morning. For Monday’s items please bring on Friday.

Annual Registration Packet Available NOW

Our Annual Regstration is now available.

All families of students enrolled at Monticello Academy for the 2018-2019 school year need to complete the registration. Instructions for completing the registration were sent out by email Wedneday, August 4th. 

  • Please make sure to read through all policies with your children. 
  • Please update your address and phone number.
  • Please make sure that you have at least three emergency contacts listed.

Online Registration & Class Selection

The school year is almost upon us. There is excitement in the air at Monticello Academy.  Teachers will be returning next week to begin preparing their classrooms and getting ready for students to return. Our building renovations are going well. We can't wait for you to see the improvements at Back to School Night.

To begin this new year we want to remind you that all parents and students must complete the annual online registration. Please be sure to review your family record ensure that parent phone numbers and address are updated as well as list at least three emergency contacts with phone numbers. You will find the Online Registration located on our school website under the Online Registration link on the left side of the webpage.

We anticipate the scheduling for grades 5-8 to begin Monday, August 6, 2018Students in grades 5-8 will be able to select classes through Compass on Wednesday, August 8, 2018 as long as the family registration is complete. Some students may already have some classes selected for them. Instructions for class selection are in your email. Please remember that scheduling is accomplished on a first come first serve basis. An email with information to set up your Compass account will be sent out later this week.

IMPORTANT NOTICE: 7th grade and all NEW students in grades 5-8 who have NOT had their updated immunization records turned in and recorded will not be allowed to select their classes until the updated records have been received and recorded. A message will be displayed in the scheduling system notifying students and parents if the records have not been recorded. Please email immunization records to: admissions@monticelloacademy.net or fax 801-417-8041.

Kindergarten and 7th Grade Immunization Records Due

State law(53G-9-302) requires that children must be appropriately immunized to attend a Utah school. Our facility supports this policy. Please obtain the required shots for your kindergarten/7th grade student(s) and provide a current immunization record to us. 

If your child is not immunized due to medical, religious, or personal reasons, you are required to provide us the appropriate Utah exemption form. If your child has immunity against the disease for which the vaccination is required because your child previously contracted the disease, you need to provide us a document from a healthcare provider verifying that. 

Possible places to take your child to receive the missing required immunization(s):

  • his/her healthcare provider
  • your local health department immunization clinic
  • or your local pharmacy.

You or your provider may fax (801-417-8041) or email (admissions@monticelloacademy.net) your child's updated immunization record. You may drop off your records in person on Wednesday, July 25th or Thursday July 26th from 9:30 am to 2:30 pm.

Please do not wait to set up your appointments for these shots as they are required for every kindergarten and 7th grade student in the state. Clinics and doctor's offices are busier the month of August. 

NOTE: Your child may not be able to access the scheduling system in August if we do not have their updated immunization record.
